Online Access to Medical Records
From 1st April 2016 Lime Grove Medical Centre offer online access to your medical record (also known as Detailed Coded Records access) which fulfils the contractual requirement for the 2015/2016 GP contract. Some examples of the coded information patients will be able to view with online access includes: vaccinations and immunisations history, consultations, and test results. We do not currently have any plans to offer Full Clinical Records online access as this service is not a contractual requirement in the GP contract 2015/16
In order to complete your registration for online access to your medical record in SystmOnline, please visit the practice to sign a consent form to acknowledge your acceptance of the risks associated with online records access.
If you do not already have electronic access for booking appointments/prescription requests, you will also have to bring with you:
- One form of photographic identification (i.e. passport or photo driving license)
- One proof of address (i.e. bank statement)
If approved, we will provide you with a username and password which will allow you to access SystmOnline to view your medical record online.
Your medical record will be reviewed by a clinician prior to online access being granted.
If for any reason you are not granted access to your medical records, you will be contacted to discuss the reasons for this decision.
