Want to make a complaint?

We aim to provide high quality family health care to all patients at the surgery but acknowledge that occasionally things do not go as smoothly as we would like.

How to make a complaint:

You have the option to either have your complaint investigated by the surgery or by the complaints team at NHS England.

If you wish the surgery to investigate your complaint:

  • You may voice your complaint to a member of our team.
  • Request a complaints form from the reception team which can be emailed or given in person.
  • Write into the surgery detailing your concerns. This can be addressed to our Patient Services Manager and can be submitted via our email ddicb.admin.lgmc1@nhs.net, in person by handing to the front desk or via post: Lime Grove Medical Centre, Lime Grove Walk, Matlock, DE4 3FD

If you wish NHS England to investigate your complaint, you can contact them via the following methods:

  • Tel – 0300 311 22 33
  • Email – contactus@nhs.net and state 'for the attention of the complaints team' in the subject line.

All complaints will be acknowledged within 3 working days and a full investigation will be carried out.
