Statement of Purpose – Lime Grove Medical Centre

Address: Lime Grove Medical Centre, Lime Grove Walk, Matlock, DE4 3FD

At Lime Grove Medical Centre, our mission is to provide high-quality, compassionate, and patient-centred care to our local community. We are a well-established GP surgery committed to delivering comprehensive, accessible, and responsive healthcare services tailored to the needs of our diverse patient population.

Our Core Services

We offer a broad range of services designed to promote health, prevent illness, and support patients in managing ongoing conditions. These include:

  • Chronic disease monitoring (e.g., diabetes, asthma, COPD, hypertension)
  • Vaccination programmes (including routine childhood immunisations, travel vaccines, and flu campaigns)
  • Sexual health services, including contraception and screening
  • Diagnostic testing, including blood tests, ECGs, spirometry, and urine testing
  • General medical consultations for both acute and long-term conditions
  • Health and well-being support, including lifestyle advice, mental health support, and social prescribing
  • Physiotherapy services and musculoskeletal assessments
  • Minor surgery procedures, including joint injections and skin lesion removals

Extended Access Appointments

To improve access for our patients, we offer Extended Access appointments through collaboration with our Primary Care Network (PCN). These are available:

  • Every Friday evening from 6:30 PM to 8:00 PM
  • Every 5th Friday from 6:30 PM to 8:00 PM
  • Every 5th Saturday from 9:00 AM to 5:00 PM

Appointments during these times are provided by a multidisciplinary team, offering flexibility for patients who may be unable to attend during regular opening hours.

Home Visiting Service & Debrief Process

Each day, dedicated appointment slots are reserved for the on-call clinician to assess patients who may require a home visit. Where appropriate, a PCN clinician is assigned to conduct the home visit. Upon completion of the visit, a formal debrief takes place with the surgery team to:

  • Ensure continuity and safety of care
  • Communicate clinical findings and action plans
  • Highlight any concerns, including safeguarding or deterioration in health
  • Facilitate appropriate follow-up or escalation

Should issues arise with the home visiting process, these will be reviewed through our clinical governance framework to uphold the quality and safety of care.
